ADULT INTEGRATED HEALTH HOME

WHAT IS INTEGRATED HEALTH?

Integrated Health Home is a program running in the Siouxland area to address whole health

for the mentally ill. As part of a total health process, Integrated Health Home works to serve the mentally ill

to both enhance and extend the life of people who suffer from mental issues. By addressing physical health and

wellness, Integrated Health Home hopes to extend long and healthy lives of the mentally ill in the Siouxland area.

INTEGRATED HEALTH HOME PROGRAM

An Integrated Health Home (IHH) is a team of professionals working together to provide whole person, patient-centered, coordinated care between behavioral and physical health providers to improve quality of care & outcomes for adults with a serious mental illness (SMI) and children with a serious emotional disturbance (SED).

Key Elements of Iowa’s Integrated Health Home Program include:

 

  • Customized “whole health” treatment plans focusing on prevention, early intervention, and wellness, including coordinated physical and behavioral health care management.

 

  • Peer support services to encourage the individual’s commitment to recovery and wellness.

 

  • Active coordination between behavioral and physical health providers to improve care.

HABILITATION SERVICES

Habilitation is a program to provide Home & Community Based Services (HCBS) for Iowans with the functional impairments typically associated with chronic mental illnesses. Habilitation Services are designed to assist participants in acquiring, retaining & improving the self-help, socialization & adaptive skills necessary to reside successfully in home & community-based settings.

Services coordinated & authorized through the IHH include:

  • Case Management

  • Home-Based Habilitation

  • Day Habilitation

  • Prevocational Services

  • Supported Employment

WHO IS ELIGIBLE?

  • You have to enroll in Medicaid

  • An adult with a Serious Mental Illness (SMI):

    • Schizophrenia

    • Schizoaffective Disorder

    • Bipolar Disorder

    • Major Depression

    • Other serious mental health condition that causes significant impairment in daily functioning.
