Let us help! We have a nurse and crisis tech at the center 24 hours a day. Our staff is caring professionals who are ready to serve the community. Our Sioux Rivers Regional Assessment Center is in conjunction with Siouxland Mental Health. If you need therapy, psychiatry, or community services, the center will help you with those services at Siouxland Mental Health or preferred agency.
WHO THIS PROGRAM IS FOR
This service is appropriate for those whose mental health symptoms can be stabilized quickly and he or she can safely return to the community. This voluntary service is for those that do not require hospitalization but need assistance and medical oversight to prevent symptom exacerbation.
Individuals are served on a voluntary basis and must be 18 or above. Also, they need to be Woodbury, Plymouth, or Sioux County residents. Please call if you have a question about a client outside this demographic.

The Stabilization Center
The Stabilization Center continues the services that clients are involved in with the community. The team of professionals at the Assessment Center will work with others in the community so our clients have all the necessary resources and services. While at the Stabilization Center, clients will be offered care coordination services, therapy services, psychiatry services, and any other needed services requested

The Assessment Center
The Assessment Center allows an individual to stay for up to 23 hours. The individual will be monitored by a group of medical and mental health professionals who truly care about your well-being. The individual will have the ability to reach out to natural supports and include them in their stabilization planning.

Our Assessment and Stabilization Center is committed to providing comprehensive care in a safe, confidential, and comfortable environment. Our services include:

Crisis screening & assessment access to medical personnel

Access to a mental health professional for consultation 24/7

Medication administration
-
Short-term therapeutic programming through group, individual, and skill-building

Care coordination to include a stabilization plan, action plan & treatment summary

Referrals to appropriate community resources

Discharge planning & follow-up services.
We want our patients to be informed and engaged participants in their treatment experience while they are here. Individuals are served on a voluntary basis and must be 18 or above. Also, they need to be Woodbury, Plymouth, or Sioux County residents. Please call if you have a question about a client outside this demographic. Referrals are accepted from law enforcement, outside providers, family, and by one’s self. An individual must meet admission requirements before they are accepted into the program.
